Anyone else here remember the radio program that IPR used to carry on Saturday nights known as the Vinyl Cafe? I always think of The Vinyl Cafe Christmas stories this time of year because they are gentle, uproariously funny and stand the test of time.
If you are unfamiliar with the Vinyl Cafe, the stories revolve around regular guy Dave, his wife Morley and their children Sam and Stephanie. They are Canadians simply living an average life. The story teller is Stuart McLean who graced the airwaves for decades with his stories before his death in 2017. I hit on the Vinyl cafe memories trying to find a respite from Trump.
We will begin with “Dave Cooks The Turkey.” I have no idea of the chronological order of these tales, but this one seems to be refernced in later stories. So let’s leave the world for a while and travel in our minds to Canada(22 minutes):

Republicans: “We gotta do something about trans people playing sports.” – Gramps
Following on my previous post concerning the looming implementation of major changes in traditional medicare in 6 states (but I think we can be assured that Republicans will push hard for full implementation quickly) I think it is time that the United States to come into the 21st century and join the world in providing health care for its citizens.
Why? First off it is the only real moral option. To leave any portion of the population without access to health care because of some failing violates every moral code I have ever heard of. Choosing who gets and more importantly who doesn’t get health care, involves making judgment on people that we are not able to make. Especially when people are denied health care because they are too poor.
Secondly, simply to keep ourselves and our families safe from infectious diseases and potentially from those with mental problems it is best for our health that such people have access to health care. Do you seriously want to be in crowded areas with people who have untreated infectious diseases because they can’t get heatlth care for some reason?
Third, it is incredibly cheaper to have a universal system that has low administrative costs for a stremlined system than to have a fragmented system with lots of overhead that is crushingly expensive. If you want to just throw your money away the US health care system is made for you. If you want to spend money wisely and get bang for your buck, spread the health care risk over the largest possible risk pool you can. That would be a national universal health care system!
From Sen Bernie Sanders: (3 minutes)
What do I want for Christmas? A National Universal Health Care System!
As the Republican Party implodes on its greed and corruption, it sure seems that next year’s election would be a great time to push hard for Democrats to campaign on a truly universal health care system. It is what the younger generation wants!
On average, other large, wealthy countries spend about half as much per person on health as the U.S.
Wealthy countries, including the U.S., tend to spend more per person on healthcare than lower-income countries. However, even among higher-income countries, the U.S. spends far more per person on health.
Comparing health spending in the U.S. to other countries is complicated, as each country has unique political, economic, and social attributes that contribute to its spending. Because health spending is closely associated with a country’s wealth, the remaining charts compare the U.S. to similarly large and wealthy OECD countries.
Make no doubt that there have been two top targets for Republicans of all stripes since the New Deal became ensconced in our country. The first, of course, is Social Security. Target numero uno since it was signed into law in August of 1935, Social Security is to Republicans the very essence of socialism. Imagine being able to quit working with dignity after a certain age without starving? What a concept.
In a very close second is the Medicare program which provides health care for the elderly. Made into law in July of 1965, Medicare saved seniors from dying way before their time due to lack of funds to pay hospital and other care needs. While it had some drawbacks such as the co-pay that Republicans insisted on, it was still a godsend to Americas elderly. Democrats hoped that a Medicare system for all would follow closely behind.
However, Republicans for 60 years have been able to stop any furtherance of Medicare, at the behest of their funders, Americans inurance companies. While Republicans haven’t been able to kill Medicare they have been able to do the next best thing and wound it in such a way that its full effectiveness has been blunted.
Now it looks like they have a weapon that could wound it to the point where it will be wounded to the point where it can be drastically altered to the point where it will look very much like the gawd awful mess we call privatized health insurance in this country. Like so many Republican initiatives, it has a name that sounds like something that favors people while actually hurting them badly.
It is called the WISeR program (Wasteful and Inappropriate Service Reduction model). This program will very much resemble both the current Medicare Advantage scam and privatized health insurance. The biggest single feature will be the introduction of pre-authorization for procedures. With current gold-standard Medicare today, there is no preauthorization. Now with real Medicare, the doctor and the patient decidewhat care is needed and Medicare is billed for the service.
Here is a brief overview of the WISeR model and why it will be very bad for seniors. (8:20)
Couple things to note here:
This is supposedly a pilot evaluation that is supposed to last 6 years.
Since Republicans make things up as they go, I think we can expect them to push to expand the program quickly claiming great (but unsubstantiated) results.
As noted the rollout will be in 6 states (New Jersey, Ohio, Oklahoma, Texas, Arizona and Washington). As I noted expect a push to expand the program quickly, so you may be drawn in much sooner than you would like.
A major note that this change was not done by legislation, but by the CMS without congressional input. However, congressional input could certainly stop and reverse this move. It must be an election issue.
As also noted in the video, there will be incentives for pre-authorizers to deny claims. This is most certainly the wrong place to put incentives.
Much of the planning and implementation for this major scam has been done while the media has been covering diversions the Trump administration has been involved in.
Once again, THIS MUST BE AN ELECTION ISSUE! Republicans now have cover to wreck the essence of Medicare without a peep.
tip of the hat to all-hat-no-cattle.com
From dwt.com we have a current list of items that have been identified as problem areas that pre-authorization will address. I am sure we can expect this list to expand greatly and quickly. Such expansion won’t take place in the light where we can at least complain, but behind closed doors at the CMS under Mehmet Oz. As with this implementation, we the people will have no say as they whittle away at Medicare.
Services Targeted for Prior Authorization
The services eligible for prior authorization under WISeR include:
Electrical Nerve Stimulators
Sacral Nerve Stimulation for Urinary Incontinence
Phrenic Nerve Stimulator
Deep Brain Stimulation for Essential Tremor and Parkinson’s Disease
Vagus Nerve Stimulation
Induced Lesions of Nerve Tracts
Hypoglossal Nerve Stimulation for Obstructive Sleep Apnea
Epidural Steroid Injections for Pain Management, Excluding Facet Joint Injections
Percutaneous Vertebral Augmentation for Vertebral Compression Fracture
Cervical Fusion
Arthroscopic Lavage and Arthroscopic Debridement for the Osteoarthritic Knee
Incontinence Control Devices
Diagnosis and Treatment of Impotence
Percutaneous Image-Guided Lumbar Decompression for Spinal Stenosis
Skin and Tissue Substitutes
Application of Bioengineered Skin Substitutes to Lower Extremity Chronic Non-Healing Wounds
Wound Application of Cellular and/or Tissue-Based Products, Lower Extremities
Providers and suppliers of the above-listed services for Original Medicare beneficiaries will have the choice of submitting a pre-service, prior authorization request through the WISeR model or going through post-service, prepayment review.
Services Have Existing Clinical Validation
Although CMS considers the above services suspect, it should be noted that they all fall under national coverage determinations (NCDs) or local coverage determinations (LCDs), including some that are subject to coverage with evidence development requirements. Rigorous evidence reviews and established standards of care are required before CMS grants an NCD or the Medicare Administrative Contractors (MACs) grant an LCD. These services have also passed the U.S. Food and Drug Administration’s “safety and effectiveness” thresholds.
We are coming up on an election year. Republicans have figured a way to destroy one of their major target programs without people hearing about it much in advance so they can do little to stop it. They have also discovered a way to destroy Medicare while their elected officials (congress members) having little blowback on it.
Let me say one more time that THESE CHANGES TO MEDICARE MUST BE ELECTION ISSUES IN 2026! Let Republicans know you will not stand for this kind of backroom dealing!
We’re keeping this email short and sweet, but thank you once again for a powerful year and for being part of this space along the way. We’re ready to keep it pushing in 2026, and we look forward to staying in the fight with you then.

Dec. 15 is the deadline to sign up on the ACA marketplace for health insurance effective Jan 1, 2025.
When I left the company and career of 25 years, securing health insurance was an issue. That was July 2009. There were no easy options, so I stayed on COBRA coverage.
The Consolidated Omnibus Budget Reconciliation Act (COBRA) gives workers and their families who lose their health benefits the right to choose to continue group health benefits provided by their group health plan for limited periods of time under certain circumstances such as voluntary or involuntary job loss, reduction in the hours worked, transition between jobs, death, divorce, and other life events. Qualified individuals may be required to pay the entire premium for coverage up to 102% of the cost to the plan. (U.S. Department of Labor website).
COBRA was expensive, so I looked around. I found the Iowa Farm Bureau offered a health insurance plan which was less expensive with reasonable coverage. More than farmers bought their plan, and so did my spouse and I. It wasn’t the best policy, yet it was good enough and met our needs.
The Patient Protection and Affordable Care Act (ACA) was signed into law on March 23, 2010. When the ACA marketplaces were organized, I completed an online form and found that with subsidies, I was eligible at a lower cost than we were paying the Farm Bureau. I signed up for a plan and stayed with the ACA until I was eligible for Medicare.
Today, with Medicare supplemental insurance costs, our health insurance bill for two people is about $935 per month, not including dental or vision coverage. I looked at buying a plan for dental, yet the cost of paying regular care out of pocket was less expensive. The same with vision. Eye treatment related to a health condition was covered under the health plan. The cost for this is slightly less than what I was paying for COBRA in 2009.
The poverty guideline for a household of two is $21,150, according to the U.S. Department of Health and Human Services. Our income is more than that, yet many struggle to bring that much home. Health insurance on such income? Without government help people can’t afford it.
All of this serves as a long build up to the significance of today.
I previously wrote the following about deadlines to sign up for health insurance on the ACA marketplace:
To be covered Jan. 1 you have to be enrolled by Dec. 15 and have paid your first premium. At this late date, I doubt Congress is going to act on the subsidies. In fact, last week, the U.S. Senate rejected extension of ACA subsidies proposed by both Democrats and President Trump. Here is from the website:
December 15: Last day to enroll in or change plans for coverage to start January 1. January 1: Coverage starts for those who enroll in or change plans by December 15 and pay their first premium. Open enrollment continues until Jan. 15 but there would be a lapse in coverage if you wait until then.
For people who don’t have health insurance now, the Dec. 15 deadline is meaningless. Even the Jan. 15 deadline can be difficult without the means to pay for a policy. There is a lot more to say on this topic, yet Tick! Tock! Life is going by at the speed of an eighteen wheeler with the hammer down.
I agree with U.S. Senator Bernie Sanders it is time to guarantee healthcare for all in the United States.
According to the most recent data, the United States spends $14,570 per person on healthcare compared with just $5,640 in Japan, $6,023 in the United Kingdom, $6,931 in Australia, $7,013 in Canada and $7,136 in France. And yet, despite our huge expenditures, we remain the only major country on Earth not to guarantee healthcare to all people as a human right. (It’s time for the US to guarantee healthcare for all, U.S. Senator Bernie Sanders, The Guardian, April 29, 2025).
President Obama was handicapped by the influence of insurance companies while he negotiated the ACA. It is remarkable any healthcare bill at all was enacted into law. Step-by-step, Republicans are stripping away the meat of the ACA, and will continue until all that is left is its bones, which they will grind up for fertilizer. Eliminating the ACA subsidies is just one part of a long plan to remove all the good things the ACA accomplishes.
If you look at my personal journey on retirement health insurance, it was only with Medicare that my worries about how I would pay a medical claim were addressed. Before that, my privileged status as a white male who was able to find a job with health insurance enabled me to find care. The care was never what I wanted, but I didn’t go broke because we had bills after our child was born in a hospital, or a major surgery.
It is easy to say there should be, as Senator Sanders says, Medicare for all. Getting that done in the United States is nearly impossible with the influence of special interests and their money in Washington, D.C. This is what makes healthcare an abomination in America. I know we can be better than this.
